Request A Curriculum Transcript

To request a curriculum transcript(s), you must click on the form below, fill it out, print it, and then sign it. An official transcript will cost $5 per transcript, an unofficial transcript is free. You can charge the fee to a credit card by calling the business office at 336-633-0292. Fax your form to the Registrar's Office, 336-629-9547. Your transcript(s) will be sent to the recipient indicated on the form by mail or it can be held at the registrar's office for pickup.
